Hims & Hers (HIMS) Q1 2026: GLP-1 Pivot Crushes Margins, Stock Plunges 15% in Premarket

Hims & Hers is one of the most dramatic premarket movers today, down 15% after reporting a quarter that revealed the painful financial cost of its strategic pivot away from compounded semaglutide toward branded GLP-1 products. The top line held up. Everything below it did not.

The Revenue Miss and the Margin Collapse

Total revenue grew just 4% year-over-year to $608.1 million — a sharp deceleration from the triple-digit growth rates the company posted throughout 2025 when compounded semaglutide was driving explosive subscriber additions. More alarmingly, U.S. revenue actually fell 8% year-over-year to $529.9 million. The only reason total revenue grew at all is an extraordinary surge in international revenue — Rest of World revenue jumped 969% to $78.2 million, reflecting early traction in new markets including the pending Eucalyptus acquisition in Australia.

The margin story is worse. Gross margin collapsed to 65% from 73% a year ago — an 800 basis point deterioration driven by the shift toward branded GLP-1 products, which carry dramatically lower margins than compounded formulations. Adjusted EBITDA fell by more than half, to $44.3 million from $91.1 million. Net loss swung to $92.1 million from net income of $49.5 million a year ago. Monthly revenue per average subscriber declined 6% to $80, reflecting the mix shift toward lower-margin products.

The GLP-1 Transition: The Root Cause

The entire quarter can be explained by one word: semaglutide. Throughout most of 2025, Hims & Hers benefited from FDA enforcement discretion that allowed it to sell compounded semaglutide — a high-margin product that drove explosive revenue and subscriber growth. When the FDA ended that enforcement discretion following Novo Nordisk's restoration of Ozempic and Wegovy supply, Hims & Hers was forced to pivot to branded GLP-1 products. CFO Yemi Okupe acknowledged the strategic pivot explicitly, noting the company expanded its assortment of branded GLP-1 products in Q1 with early demand signals showing consumer reach broadening. But branded products mean lower margins, higher cost of goods, and a completely different unit economics profile. The market is repricing that reality this morning.

Subscribers grew 9% to nearly 2.6 million — showing the platform still has consumer pull — but the revenue per subscriber decline and margin compression suggest the economics of each new subscriber are structurally weaker than before.

Raised Revenue Guidance, But EBITDA Tells the Real Story

Hims & Hers raised full-year 2026 revenue guidance to $2.8 to $3.0 billion — a significant increase that reflects confidence in the international expansion and new category launches. Q2 revenue guidance of $680 to $700 million implies sequential acceleration. But the EBITDA picture tells a different story: full-year adjusted EBITDA guidance of $275 to $350 million implies a 10-12% margin for the full year, while Q2 EBITDA guidance of $35 to $55 million implies a 5-8% margin — a continued compression before any recovery. Free cash flow of $53 million was the one genuinely clean data point in the quarter, roughly flat year-over-year.

The Long-Term Vision vs. The Near-Term Pain

CEO Andrew Dudum struck a characteristically bullish tone, reaffirming 2030 targets of at least $6.5 billion in revenue and $1.3 billion in adjusted EBITDA, and positioning Hims & Hers as building toward the world's largest consumer health platform. The pending acquisition of Eucalyptus — an Australian digital health company — extends that international ambition and is excluded from current guidance. New investments in comprehensive diagnostics and technology infrastructure are being framed as the foundation for long-term competitive advantage and cost structure improvement.
